Colobomatous Optic Neuropathy
What is it?
Congenital cleft in optic disc tissue from faulty embryogenesis
Often misdiagnosed as glaucoma
Other congenital forebrain and non-brain anomalies are often present
What does it look like?
Optic disc excavation extends inferotemporally
Cleft may also involve the retina, choroid, upper eyelid, iris
Can affect one eye or both
Associated non-progressive visual field defect
What else looks like it?
Glaucoma
Tilted optic disc of myopia
Large physiologic cup
What should you do?
Suspect optic disc coloboma if there are clefts in the eyelid or iris
Distinguish from high myopia, glaucoma
Order brain imaging to detect other forebrain anomalies
Be aware of the association with papillorenal syndrome
What will happen?
Optic disc and visual field defects will be lifelong and stable
Clinical outcome depends on other anomalies
Trap:
optic disc coloboma is often misdiagnosed as glaucoma, initiating unnecessary treatment
